About the Role:

Our client is seeking dedicated Social Workers to join their Adult Social Care team. As a Social Worker, you will play a crucial role in promoting and maintaining the independence of vulnerable adults while ensuring their safety and well-being. You will assess individual needs, formulate person-centered care plans, and coordinate support services for both service users and their carers.

Key Responsibilities:

Assess the needs of individuals and carers in line with Care Act (2014) criteria.

Develop and implement person-centered care and support plans.

Supervise and support staff in accordance with Council procedures.

Initiate and coordinate care plans in collaboration with professional colleagues and agencies.

Monitor the efficiency and effectiveness of care plans and services.

Review and evaluate the needs of service users and carers.

Maintain accurate client records in compliance with Council policy.

Skills & Abilities Required:

Proficiency in conducting strength-based assessments.

Excellent communication and interpersonal skills.

Ability to undertake safeguarding enquiries.

Sound understanding of Adult Social Care legislation and practice.

Capacity to chair meetings and work collaboratively in a team.

Effective organizational skills and ability to prioritize workload.

Qualifications & Experience:

Social Work Degree or equivalent qualification.

Registration with Social Work England for qualified Social Workers.

Relevant experience in community and/or hospital settings.

Familiarity with relevant Social Care and Health legislation.

Experience working across various agencies.

Special Requirements:

Ability to travel as required.

Enhanced Disclosure and Barring Service (DBS) disclosure is mandatory.
